MDS Nurse Coordinator (RN or LPN)
Bay Path Rehabilitation & Nursing Center is seeking a part-time (20 hours) , ON-SITE, MDS Nurse Coordinator (RN or LPN) to support our clinical reimbursement team. Requirements for MDS Nurse Coordinator (RN/ LPN):
· Experience in MDS completion required
· Certification as a Resident Assessment Coordinator (RAC-CT) preferred
· Ability to monitor, evaluate, and manage plans of care for in-house residents and patients
· Excellent communication skills with residents, families and the interdisciplinary team
· Strong computer skills, including Microsoft Office, EMR systems, etc.
· 1-2 years experience in long term care
· Active, unencumbered licensure as a registered nurse (RN) or licensed practical nurse (LPN) in the state of MA
· Prior experience with PointClickCare (PCC) required.
Job Duties for MDS Nurse Coordinator (RN/ LPN):
· Assess and monitor proper treatment for residents in nursing homes.
· Determine the health status, care plans, and procedures for intake of residents according to state and federal standards.
· Perform clinical assessments
