Compass Healthcare Consulting & Placement is conducting a search for an experienced RN Director of Patient Services - Director level Registered Nurse, RN for a Licensed Home Healthcare Agency with office located in Brooklyn, New York. Agency is very interested in a DPS who understands Licensed Home Care Programs; Clinical Compliance, Program Performance and clinical management of a home care agency. DPS will also know how to provide continuity of care in the Community..

RN Director - DPS - Licensed Home Care Service Agency (LHCSA)

Summary:

Plans, develops, implements and evaluates all of the clinical functions to market and deliver the clients services of the Licensed Home Health Agencies.

Responsibilities

Clinical Leadership & Patient Care:

Oversee the coordination and delivery of patient care services, ensuring that all documentation and care plans meet the needs of each patient and are aligned with regulatory requirements.

Monitor and evaluate patient care services, ensuring adherence to agency policies and industry best practices.

Lead and supervise clinical staff, including nurses, therapists, home health aides, and other direct care providers.

Provide guidance and support to clinical staff on clinical issues, patient care concerns, and best practices for service delivery.

Regulatory Compliance:

Ensure the agency is in full compliance with all state, federal, and local regulations.

Maintain and ensure proper documentation of all patient care activities, following agency policies and regulatory standards. o Prepare for and manage all audits and inspections by regulatory agencies, ensuring compliance is continuously met.

Quality Assurance & Improvement:

Implement and oversee quality assurance programs to monitor patient care outcomes, staff performance, and service delivery standards.

Lead ongoing quality improvement initiatives and projects to enhance patient care and operational efficiency.

Develop and monitor key performance indicators (KPIs) to assess the agency’s patient care delivery and staff productivity.

Staff Management & Development:

Recruit, hire, train, and supervise clinical staff, ensuring that they are properly licensed, credentialed, and compliant with all relevant standards.

Conduct regular performance evaluations, providing feedback and guidance to staff on professional development.

Foster a collaborative and supportive work environment to ensure a high level of job satisfaction and retention among clinical staff.

Patient and Family Communication:

Serve as a point of contact for patient and family concerns, addressing complaints and resolving issues in a timely and professional manner.

Educate patients and families about their rights, care plans, and available resources.

Ensure that patients’ needs and preferences are respected in the development of care plans and service delivery.

Operational Support:

Collaborate with the agency’s leadership team to develop and implement policies and procedures that support the effective and efficient delivery of services.

Assist in development and planning for patient services, ensuring efficient resource allocation.

Provide leadership in emergency management, including disaster preparedness and response as it pertains to patient care.

Documentation:

Ensure that patient care documentation is accurate, timely, and compliant with payer requirements, including Medicaid, and Long Term Care insurance providers.

Qualifications:

• Current registration with the New York State Board of Nursing

• A minimum of 5 years’ experience in Home Care Preferable in Staff development

• Knowledge of local, State, and federal laws and Joint Commission Standards for Home Care.

• Ability to work under pressure.

• Ability to communicate effectively in both the oral and written form.

• Excellent Interpersonal skills
